I thought I would start a single thread here to capture the LED status indicators for when a box fails, so that perhaps we can capture them over time.......

Green light blinking 3 times, box reboots

Communication problems with the IDE adapter.  Could be a loose connection (ribbon cable) or failed hard drive.

Yea, I did think it was a bit odd that there was nothing lurking around.  I did a pretty comprehensive search across the forums and nothing showed up...

Well here's a couple for you:

5: Error writing byte
6: Unable to read sector 0
7: No FAT signature (0xAA55) at end of first 512 bytes
8: I think it means no FAT16 or FAT32 partitions found - is followed by blinking LED of death
9: Unable to read a sector - I think the sector is the first sector of partition but not sure
10: no FAT signature (0xAA55) in the sector read in 9 - is followed by blinking LED of death
11: Unable to read a sector (drive Model+Serial?)

on mine the red light stays constant and the green light blinks every five seconds as long as the radio is on and a little bit after.
Head unit says no cd, any help?

That sounds like normal operation... but without any music on the keg.
