EAC for extraction to FLAC.
Foobar for converting voice prompts.

Interestingly enough, I use PMM for ripping the CD direct to the DMS [and that's probably why my Head unit display is all-sides up].
However, I also use WMP.

If it's a live stream/DJ set of my own - I also use FairStars Audio Converter to drop the bitrate/change stream type.

I use Itunes, because it sucks...  I want to have my music in itunes db, and phatnoise db.  If i am ripping alot, it is a pain to import into Itunes, so i just rip with itunes, and import into PMM.  much smoother this way.
Unless I want FLAC, then I use PMM.
I was also using Mediacoder for awhile, which is nice for transcoding into OGG.  I moslty use that now for Video.

When I ripped the bulk of my CD collection to MP3s a few years ago, I used MusicMatch Jukebox Plus.  At the time its CD ripping was the fastest by far, though the rest of the program was bloated and buggy.

Speaking of bloated programs, what I use now is Nero, which is also very fast.

I've now switched to PhatHack Media Manager for CD Ripping.
Does exactly what I want it to - but then I am also biased, as I wrote it ;)
